The authors have addressed my comments from the original manuscript. I recommend publication, but would like to add the following comments:
- Due to the large number of equations it is hard work to read the paper. To be more concise and focus on the essentials, I suggest that the authors consider moving the derivations from Section 2.2 to the appendix. The final equations for each case can be stated in the body of the text, but the derivations would be better placed in the appendix in my opinion.
