Permissions required

A template permission request form is attached below. Please modify the form as necessary and send it to the authors and publishers you need to contact, in order to obtain permission. Once you have received the permissions, please forward all of the responses to us for our records.

We are unable to publish any article containing previously published material until all the relevant evidence of permission has been received. As this may take some time, we recommend that you start as soon as you can.
