探讨血清DPPIV浓度与慢性阻塞性肺疾病(COPD)不同疾病状态下的相关性,分析其对COPD病情的判断能力和诊断准确度,为COPD筛选血清标志物。方法 收集包头市中心医院呼吸科2013年1月至2013年12月门诊和住院的病例,诊断为COPD的急性加重期患者42例为AECOPD组,经相应治疗,临床症状缓解后,全部归为COPD治疗缓解组。已超过3个月无急性加重的稳定期COPD随访患者32例为COPD稳定组。包头市中心医院体检中心随机选取中老年健康者29例为健康对照组。采用酶联免疫吸附实验法测定DPPIV浓度。比较健康组、COPD稳定组、AECOPD组血清DPPIV浓度差异。比较AECOPD组治疗前、后血清DPPIV浓度差异。观察DPPIV浓度与年龄的相关性。采用ROC曲线分析法评价DPPIV浓度对COPD的诊断价值。结果 COPD稳定组、AECOPD组、COPD治疗缓解组血清DPPIV浓度较健康对照组明显降低(P<0.001)。AECOPD治疗后DPPIV浓度较治疗前升高(P<0.001)。DPPIV浓度与年龄、性别以及吸烟史无相关性(P>0.05)。 AECOPD和COPD稳定期,基于DPPIV浓度的ROC曲线下面积均超过0.9,特异度高达0.931,灵敏度达0.75以上。结论 COPD患者血清DPPIV浓度明显降低。血清DPPIV浓度与性别、年龄无关,对COPD有很高的诊断价值,为COPD疾病的一种很好的血清学标志物。血清DPPIV为COPD的保护性因素。

After having discussed the correlation between the concentration of DPPIV in the serum and different morbid states of COPD, the concentration’s ability of providing evidences for diagnosing the disease and its accuracy are analyzed in this paper so as to select serum marker for COPD. Based on collected case data of both outpatients and hospitalized patients from pneumology department of central hospital in Baotou City from January to December, 2013, 42 cases diagnosed with acute exacerbation patients of COPD are marked as AECOPD group, and after having received corresponding treatment and clinical symptoms eased up, the same 42 patients are marketed as treated group of COPD, and 32 visiting patients who haven’t caught acute exacerbation symptom for over 3 months are marked as stable group of COPD, and finally, 29 middle-aged and elderly and healthy persons randomly selected from Physical Examination Center (PEC) of the hospital are marked as healthy control group. Then, measure the concentration of DPPIV with the enzyme-linked immunosorbent assay (ELISA) method, compare the concentration difference among persons from healthy control group, stable group, AECOPD group, and the concentration difference before and after the corresponding treatment as well. Observe the correlation between the concentration of DPPIV and the age, and evaluate the concentration’s ability of providing evidences for diagnosing the disease-COPD with ROC curve analytical approach. The results show that, compared with people from healthy control group, concentrations of DPPIV of patients from stable group, AECOPD group, treated group are much lower ( P<0.001), and after having received treatment, the concentration of the patients from AECOPD group have increased (P<0.001), and the concentration is unrelated to age, gender and smoking history (P>0.05). In addition, the area under the concentration-based ROC curve has reached 0.9 and the degree of specificity 0.931and the degree of sensitivity above 0.75. In a word, concentration of DPPIV of COPD patient is lower than healthy person, unrelated to age and gender, and thus can serve as a good serum marker matter and can provide rich evidence for diagnosing COPD. DPPIV in the serum serves as a protective matter for COPD.

【Objective】The purpose of the present study was to explore the correlation between serum DPPIV and chronic obstructive pulmonary disease (COPD) at its various disease states, analyze its applications in the prediction and diagnosis of COPD disease, and test the possibility of DPPIV as the serological marker for COPD by screening. 【Methods】 Samples from outpatients and inpatients were collected from Pulmonary Department, Baotou Central Hospital, from January 2013 to December 2013. 42 cases were initially diagnosed with acute exacerbation of COPD, thereafter were called AECOPD group in this study. After appropriate treatment, clinical remission occurred in all patients and therefore were classified as COPD remission group. Those 32 patients who have been followed up and been stable for more than three months without acute exacerbation were COPD stable group. 29 mid and elderly healthy subjects randomly selected from Physical Examination Center, Baotou Central Hospital were used as healthy control group. DPPIV concentration was measured by enzyme-linked immunosorbent assay (ELISA) and the difference in serum concentration of DPPIV was compared among healthy group, COPD stable group, and AECOPD group. The difference in serum DPPIV concentration of AECOPD group before and after treatment was also compared. The correlation between DPPIV concentration and age was analyzed. The diagnostic value of DPPIV concentration of COPD was evaluated by ROC curve analysis. 【Results】 Serum DPPIV concentration was significantly lower in stable COPD group, AECOPD group and COPD remission group as compared with that in healthy control group (P<0.001). Serum  DPPIV concentration in AECOPD group was increased after treatment as compared with before treatment (P<0.001). There was no significant correlation between DPPIV concentration and age, gender, or smoking history (P> 0.05). For both AECOPD and stable COPD, the under curve area in ROC, which is based on DPPIV concentration, was more than 0.9, with a specificity of up to 0.931 and sensitivity of greater than 0.75. 【Conclusions】 Serum DPPIVP concentration in patients with COPD is decreased significantly and there is no correlation between serum DPPIV concentration and sex or age. Serum DPPIV is not only a protective factor, but also is of high value as a good serological marker for the diagnosis of COPD.
