| 难治性心力衰竭又称顽固性心力衰竭，指心功能3到4级的充血性心力衰竭患者，应用适当的病因治疗和规范化抗心力衰竭治疗2周以上，心力衰竭的症状和体征未能得到改善甚至恶化者。此阶段患者往往表现为不能平卧，肺部啰音，心率增快，双下肢水肿，甚至腹水、胸水、尿少，四肢厥冷，发绀，在洋地黄未能达到治疗量时即出现中毒症状。中医方面认为心衰的病理机制为心气虚、心阳虚为本，淤血内停为标。治以益气、温阳、活血、化痰、利水之法。选黄芪甘温，补气以帅血运行；桂枝辛甘，温阳化气，温通血脉；泽兰、益母草活血祛瘀，兼能利水，葶苈子泻肺平喘，利水消肿，重在泻肺中水气，痰涎，邪盛喘漏不得卧者为宜；茯苓味甘而淡，甘则能补，淡则能渗，既可祛邪，又可扶正，利水而不伤正气。共用具有补气温阳，活血祛瘀，泄肺平喘，化痰祛湿，利水消肿之效。拟达到临床有效率95%。 |

New York Heart Association classifieation 有关心脏病患者体力活动的机能及治疗分类 Refractory heart failure , also known as intractable heart failure , refers to New York Heart Association functional class III to IV congestive heart failure patients whose signs and symptoms of heart failure has not been improve or even worsening after two weeks of appropriate treatment and standardized anti- heart failure therapy .Patients at this stage can not be supine and manifested pulmonary rales , increased heart rate , lower extremities edema;sometimes even ascites, hydrothorax, oliguria, cold limbs , cyanosis , and subtherapeutic digitalis dose developed signs and symptoms of digitalis poisoning . 先翻到这，中医部分一会再翻。 |

Thus heart failure therapy should be replenishing qi, warming yang, promoting blood circulation, reducing phlegm and inducing diuresis. Nevertheless, radix astragali was warm in property and can nourish one’s vitality .Cassia twig was acrid-sweet in flavor and can warm yang to activate qi ,as well as promote blood circulation. Eupatorium japonicum and leonurus artemisia can promote blood-circulation, remove blood-stasis, and induce diuresis. Pepperweed seed can remove heat from lung , relieve asthma and induce diuresis to alleviate decema; especially remove the hydrosphere and phlegm in lung .It is suitable for those who is suffocative and can not be supine. Poria cocos Wolf was slightly sweet in flavor and can remove pathogen and nourish the vital QI, induce diuresis and conserve vital QI. The combination of all these can enhance vital QI ,warm yang ,promote blood-circulation, remove blood-stasis, remove heat from lung, relieve asthma, reduce phlegm, remove hydrosphere, induce diuresis and alleviate edema.
