求专业英语的同仁，帮忙翻译一份问卷，论文急用！

由于论文需要，急需翻译一份外文问卷，本人英语超烂，希望英语底子好的，帮忙翻译一份问卷，39个题目，不甚感激！！论文急用！ Please circle the number that corresponds to the degree to which you agree with each statement. 1.
