[求助] experimental biology and medicine(EBM)修回意见，急求帮助！

今天收到experimental biology and medicine(EBM)杂志的返修稿，之后再进入正式的审稿程序，首次批改中注明了一些问题，本人不是很懂，拿出来请高手给予指点，非常感谢！

1、标题批注如下“This paper needs a thorough revision for English grammar and usage.
A brief selection of points needing attention has been highlighted in the text for your information.”
2、“A Mandatory Submission Form is required. ”他们给我一个PDF文件，需要作者签字还是怎么着呢？
3、“There is no Figure Legend page。”似乎是在图表说明的问题，可是我已经做了图表说明，不知道他们说的是什么意思？
4、摘要部分：“Abstract: An abstract of no more than 300 words must accompany all articles and should not be structured. ” 不懂什么意思？
5、Drug部分：“convert list into prose form.”，我只是罗列了药品名称，括弧里面注明哪个厂家的，他们是不是让我把他们写成句子形式啊？同样的还有Instrument部分说“You need to re-write this in full sentence form, not just as note-form.”
6、在Observing Indexes部分之前他们给修改的语法都很详细，不过其后就一点都没有修改，不知道他们是让我自己修改还是没有错误，我觉得没有错误似乎不可能啊，可是他们为什么没有给我全部修改完呢？标题批注上还说了“This paper needs a thorough revision for English grammar and usage.
A brief selection of points needing attention has been highlighted in the text for your information.”
是不是后面的是让我自己修改啊？

因为我是第一次投稿，经验不足，好多不是很了解，请高手赐教，不胜感激！！
