Year: 1997 Abstract: Earlier research has supported the hypothesis that a P300 (P3) event-related potential (ERP) based procedure can be used to determine whether intact recognition memory is present when a person attempts to simulate a memory disorder. This hypothesis was further tested in a series of 3 experiments, the ultimate goal of which was to determine the validity and utility of the procedure. One study examined whether the relationship between P3 amplitude and recognition memory reported in undergraduates would also be observed in the population most relevant to the diagnosis of malingered memory deficits, that of closed-head injury (CHI) patients. It was found that when CHI patients have intact recognition memory, P3 amplitude can be used as a measure of intact recognition memory in this population. In the second study, an attempt was made to model the behavior and ERPs that amnestic patients might demonstrate with relatively easy recognition memory tests, by increasing the difficulty of a re cognition memory test taken by memory-unimpaired undergraduates. The pattern of ERP activity that denotes intact recognition memory was eliminated in the difficult condition of the test. All of the subjects in the difficult "amnestic" condition were correctly classified as not having intact recognition memory based on intra-individual tests (specificity = 100%). In the third study, the sensitivity of a P3-based procedure for detecting malingering based on a forced-choice recognition test for digits, was compared to the sensitivity of a behavioral test of malingering developed for use with the Booklet Category Test. Because of statistical limitations, it was not possible to determine if the sensitivities of the two tests differed, but results suggested that ERP and behavioral tests demonstrate greater sensitivity when used in combination, than when either test is used alone. The results of Experiments 1 and 2 support the utility of a P3 based procedure for the detection of malingering b y demonstrating the procedure to be a valid measure of intact recognition in CHI patients, and a valid measure of impaired recognition in a model of amnesia.
