Note: submitting the tabulation filled the by applicant under the approval of the one in charge of his department, also the test cost and related approval determined by the analysis and test center along with contacting external test department, and submitting it to the dean ofesearch institute for the appoval ultimately.

Note: The form is filled by the applicant and submitted to the Analysis and Testing Center after approval by the head of department. The analysis and Testing Center is responsible for contacting the external testing facility, the cost of testing, the approval, and the submission to the Dean of Research Institute for final approval.
