请教decision letter中的‘in the appropriate box on the form’是什么意思？ 已有2人参与

在decision letter中editor写了这么一句，说的意思似乎是要将所有的response搞到box中，请教这是什么意思哪？我已经写好了response，是否还得画个表格并注明这个response对应改动了文章的那一段那一句？求例子。多谢多谢 IMPORTANT: When you submit your revision, please detail the changes you have made to the paper and respond to all of the reviewer, Editorial Board, and Managing Editor comments in the appropriate box on the form.
