The first volume of the official report of the Columbia Accident Investigation Board chaired by Admiral Harold W. Gehman, Jr. Also referred to as the CAIB Report or the Gehman Report. Not to be confused with later reprints--this is the full size 1st public printing. Printed in full color, this volume covers the evolution of the space shuttle program, Columbia's final flight, accident analysis, other factors considered, why the accident occurred, from Challenger to Columbia, decision making at NASA, the accident's organizational causes, history as cause, implications for the future of human space flight, recommendations and Appendices A-C.
