[求助] 求英语专业的同仁，翻译一份问卷，论文急用！

由于论文需要，急需翻译一份外文问卷，本人英语超烂，希望英语底子好的，帮忙翻译一份问卷，39个题目，不甚感激！！论文急用！

太长了，只能完成一部分，而且翻译的质量好像也不过关。。不过内容貌似不是太难，待来者。
对下面每句陈述，根据你的认同程度圈出相应数字。
1、我十分担忧专业和职业选择。
