[求助] 投SMJ，他们要求要提交一个 Contributorship Agreement 没弄过求高手指点

最近投SAUDI MEDICAL JOURNAL 杂志，在线投完后编辑回信说需要填一个Authorship, Contributorship Agreement & Publishing license  。具体情况如下

本人做的关于细胞方面的实验。不知道这个表格中，通讯作者、作者的贡献该怎么填合理，求高手指点。不胜感激
