求助elsevier下的medical hypotheses 投稿系统！！急

如题 进行到addtional information 这一步了 提示：Have you submitted your title page? Your title page should be the first page of your manuscript. The title page should give the following information: (1) title of article; (2) initials and name of each author, with highest academic degree(s); (3) name and address of the department or institution to which the work should be attributed; (4) name, address, telephone and fax numbers and E-mail address of the author responsible for correspondence and to whom requests for offprints should be sent; and (5) sources of support in the form of grants. 文章标题和作者信息之前已经输过了 剩下的地址通讯作者地址 应该输入到哪里呢 是它提供的那个小框框吗 但是字符数限制在200以内，怎么都会查出啊 应该这么办呢 急等~~！！  _%Z@P1@YY5HES~`_BMT@ST3.jpg |
