[求助] 请问checklist of documents required for J-1 Student Intern DS-2019 request

请问各项要求的确切意思，以大家的经历，是否要求每一项都必须都准备？没有的怎么办，比如托福考试，我就没参加过，难道就为了做“访问学者”，再花钱去参加一次？
Prospective Intern Documentation
[ ] Documentation in English of enrollment and good standing from current institution - can be a letter written on official university stationary/letterhead
[ ] Signed letter of approval for participation in J-1 Student Intern program from the student’s home institution’s dean or academic advisor
[ ] Documentation of funding support in English for the intern and any dependents, if the internship is an unpaid position at Washington University. Documentation must be original, in English, and the amount must meet the minimum yearly living expenses determined by OISS.
[ ] Agreement to terms of internship participation (signed by student)
[ ] Written statement from prospective intern describing how the internship will assist in fulfilling his/her educational objectives and academic course of study
[ ] Documentation of health insurance coverage for the intern and any dependents for the entire period of the internship
[ ] Signed Health Insurance Supplementary Statement
[ ] Official TOEFL scores with a minimum score of 550 and administered within 2 years of the internship start date
[ ] Copy of passport
[ ] Signed Form B printed from e-scholar
*Upon review of submitted documents, OISS may ask for additional information or documentation from the department or the intern.
