[求助] 申请DS-2019表疑问

我正在申请DS-2019表。对方让我填写一个screening  form。我在最后签了名。结果对方说不对。他是不是让我把“Signature”上面的一句话抄上去。请哪位有经验的虫友看看。
他的邮件原文是：
Please note you have to sign the attached (a signature is needed) writing your name is not acceptable.
Once you have signed, please resend.

screening  form格式如下：

I certify that the information provided above is true and complete.  I further certify that, to the best of my knowledge, I am not charged with a criminal offense related to any Federal Health Care Program, or proposed for exclusion.

I understand that false or misleading information given in my employment application, interviews, or this form will render my application void and will be just cause for termination of my employment.  I authorize you to review my eligibility to participate in federally funded health care programs, including any investigations as are necessary in arriving at an employment decision.  I understand that the University of Chicago may, upon execution of this authorization, investigate the information contained in my employment application and/or background information, the results of which will be used as a factor in making employment decisions.
