老外给了一份邮件关于办签证，不知道其中的第5条的name of affiliation是什么意思，请求有经验的虫友给与解答，谢谢！

Dear ICOMAT participants who need VISA for Japan,
If you are coming from a country whose citizens are required VISA for
Japan, please provide us with following information. We will prepare
documents you need for VISA application and send them to you by courier
service.
1) Full name as shown on your passport
If you have name in Chinese character, please also write it.
2) Date of birth　DD/MM/YYYY
3) Gender
4) Nationality
5) Name of affiliation and your title
6) Flight schedule to/from Osaka, Japan (including flight number and
airport) You can change the schedule later if necessary. Please tell us
your tentative plan.
7) Name, address and phone number of accommodation you will stay in
during the conference
8) Mailing address to which documents for VISA application are to be sent
