CAUSE AND MECHANISMS OF INJURY
Hasler contends that 96 per cent of all supracondylar fractures of the humerus are extension-type injuries that occur during falls onto the outstretched hand, while between 1 and 2 per cent occur through falls onto the olecranon with the elbow flexed.
  It is unclear how these figures are reached, because Hasler describes no study into the incidence and mechanism of such injuries. However, Wilkins, a much quoted and well respected author on the subject, lends support to this notion, claiming that between 90 and 95 per cent of these injuries result from extension and the rest from flexion.
  Extension type injuries occur when the elbow is in a position of hyperextension with the olecranon acting as a fulcrum across which the humerus breaks.
  The peak age of injury is usually quoted as between four and eight years, This distribution is explained by Nork eta/: bone is weaker at this stage of development and this, and the ligamentous laxity predisposing the elbow to hyperextension, render the elbow friable.
  Many ot these injuries occur during play, sport or during falls from a height. This again fits with the stage of development, this time from a social perspective, as children of this age have little perception of danger and engage In risky and adventurous activities and play.
  Interestingly. Waltzman et a/ find that, even with heightened parental supervision, the incidence of injury was the same. It seems, therefore, that it is 'natural ' for children to sustain this kind of trauma at this age.

原因及损伤机制
Hasler 认为96%的髁上骨折是发生在跌倒时伸在外测的手上的扩展型受伤,只有1%-2%的是由于跌倒时的肘部弯曲,

目前还不清楚这些数据时如何得到的,因为Hasler并没有介绍任何关于此类伤害的发生率和机理的研究。
然而在这个领域很受人尊敬的著名学者Wilkins支持这种观点,他认为90%-95%的此类受伤是由于伸张,剩下的是由于弯曲。扩展型伤害发生于肘部以鹰嘴为支点的旋转时造成的肱骨损坏。受伤的高峰期是4至8岁。

Nork对此的解释是:骨骼在这个年龄段很脆弱,韧带松弛是导致肘部过伸和骨折的原因。很多这类受伤发生在玩游戏,运动或者从一定高度摔下时。这也和孩子在这个时期的发育相吻合,从社会的角度来看,这个时期的孩子缺乏危险感,会在有危险的活动中玩耍。
有趣的是。 Waltzman等发现,即使是父母提高监督程度,孩子受伤的发生率还是不变的。因此,这个年龄段的孩子受这种伤是很自然的。
