是表格填写问题哦！ 请问应该选哪个？ YOU MUST CHECK ONE OF THE FOLLOWING: 1 I have received confirmation in writing that my paid appointment at USC will include access to USC health insurance through the USC Benefits Office. I understand that upon my arrival at USC, I will be required to choose the State Health Plan in addition to the required supplemental medical evacuation and repatriation plan. 2 I have received confirmation in writing that the department at USC with which I will be affiliated will purchase alternate coverage for the required health insurance. 3 I have insurance coverage that is backed by the full faith and credit of my home government and have attached the required verification in English. 4 I have already purchased a plan that meets the above requirements and have attached a copy of the policy to this form. (Must be in English or an English translation must be attached.) 5 I will purchase insurance coverage that meets the above requirements and bring evidence of this insurance to International Support for Faculty and Staff upon my arrival in the United States.
