三．脂类与特应性疾病 1. 临床表现 饮食中脂类成份引起特应性疾病,往往不具有特异性,并且存在漫长的潜伏过程。所以仅引起少数学者们注意。特应性疾病早期表现往往是婴儿湿疹,继而出现鼻炎和哮喘。这此情况多出现在atopy 患者,即带有家族遗传倾向。同一患者具有上述3 种疾病,在临床亦不少见。还可见到循环的现象 ,即同一患者具有湿疹、鼻炎、哮喘,但不是同一时间发作。有着此起彼落的表现。这说明它们可能存在一种共同的致病因素,只是不同时期表现不一样而已。Atopy 患者早期出现的湿疹,普遍认为与饮食中因素关系密切。所以学者们对婴儿的喂养饮食的内容作了一系列研究。发现患特应性疾病婴儿其母乳含较多的亚油酸和较少的多不饱和脂肪酸。在人工喂养中,大部分研究发现:豆奶喂养较牛乳喂养更少患此病。这些事实表明:饮食中脂类成份在特应性疾病早期形成阶段,具有重要作用。此外,饮食中脂类也参与其发病过程。如:油煎后的食品,引起哮喘患者气道反应性增强;牛奶去脂后,引起患者肺部的气体交换明显增强。湿疹常见皮肤干燥、鱼鳞病等皮脂分泌障碍。所以不论在特应性疾病的早期形成和其后的发病过程中,饮食中脂类因素都是一种不可忽视的致病因子。在治疗方面:用鱼油可以减轻哮喘和湿疹患者的症状。并发现尿中减少52系列白三烯排出。提示与拮抗花生四烯酸有关。 |
