5.1 生产全过程中，由于人员、原料、工序、设备、违反操作等原因造成的原料、中间产品和成品的数量损失、质量损害以及设备、仪表的损害均视为异常情况。 5.2 发生异常情况的有关车间，必须及时采取有效措施，避免异常情况扩大，并积极挽救，减少损失。 5.3 凡发生异常情况，最先发现者（当事人）应立即向车间主任报告，而后逐级上报到生技部、质保部，重大异常情况可立即向质量保证部报告，按照《事故管理程序》（SOP No.RY/EHS/034(R1)）或《偏差调查管理程序》(SOP No.RY/ZL/010(R1))的规定处理。 5.4 发生异常情况的车间负责人应责成有关人员按规定填写《异常情况处理单》（见记录一），报告质量保证部，以上交报告单时间开始计时，处理一般异常情况时间不超过三天，处理重大异常情况不超过七天。 5.5 《异常情况处理单》、《异常情况处理报告》纳入本批的批生产记录。 |
