重建股骨距人工股骨头置换术治疗高龄股骨粗隆间骨折 【摘 要】 目的 探讨人工股骨头置换治疗高龄不稳定性股骨粗隆间骨折的手术方法及疗效。 方法 回顾分析2005年10月~2007年10月采用两种治疗方法治疗的113例高龄不稳定性股骨粗隆间骨折，采用复位固定股骨大、小粗隆，修复稳定股骨近端，重建股骨距结合骨水泥型人工股骨头置换术治疗58例（实验组）；采用切开复位r钉内固定术治疗55例（对照组）。实验组男21例，女37例；年龄平均75.0 岁。骨折按Evans-Jenson分型：ⅢA型10例，ⅢB型16例，Ⅳ型32例。对照组：男17例，女38例；年龄68～87岁，平均72.5岁。骨折按Evans-Jenson分型：ⅢA型13例，ⅢB型14例，Ⅳ型28例。两组均合并骨质疏松，合并冠心病、高血压病、糖尿病、慢性支气管炎等慢性疾病一种或几种；无其他部位骨折。比较两组患者术前的基本情况，手术时间、术中出血量、术后下地时间、髋关节功能评分及并发症。结果 两组一般资料比较差异无统计学意义（P>0.05）；两组患者均顺利完成手术，术后切口均I期愈合；实验组手术时间、术中出血量、输血量均明显少于对照组（P<0.05）。两组患者均获随访，随访时间1.5~3.5年，平均2.6年；实验组并发症发生率3.4%，对照组16.4%，（P<0.05）。术后12个月，按照Harris 标准评定髋关节功能，实验组优良率93.1%，对照组70.9%，差异具有统计学意义（P<0.05）。结论 人工股骨头置换治疗高龄股骨粗隆间骨折的临床效果良好，稳定近端，重建及修复股骨距是的手术关键。 【关键词】 重建股骨距 人工股骨头置换术 高龄 股骨粗隆间骨折 The Treatment of Unstable Femoral Intertrochanteric Fracture in Elderly Patients with The Proximal Stability and The Bone Cement Prosthesis（文题字母改大写） 【Abstract】 Objective Introduce the treatment of intertrochanteric unstable fracture in clinical aged patients with stabilizing proximal portion,rebuilding calcar femorale, replacement of artificial long-stem bipolar femoral head with bone cement. Methods 113 aged patients with intertrochanteric unstable fractures were treated ，58 casas by replacement of articial long-stem bipolar femoral head with bone cement after reduction and fixation of femoral trochanteric fracture, rebuilding calcar femorale, 55 examples adopt inner fixation treat .To compare between two groups after operation time , bleeding amounts , blood transfusion amounts , time of weight loading after operation, the complication occurrence rate ,the effects of all cases in recent period by the system of Harris. Results Displacing group of operation time , bleeding amounts , blood transfusion amounts , skill time , the complication occurrence rate are lower than the fixed group of inner equally , standards of grading appraises the fixed arthrosis function good rate group higher than inner , difference has statistics meaning according to Harris. According to the system of Harris: the excellent and good rates are 93.1%,and no one occured infection,dislocation,prosthetic loosening, periprosthetic femoral fracture after treatment. Conclusion The aged patients of intertrochanteric fracture could do function exercise in recent period and avoid operation once by stabilizing proximal portion,rebuilding calcar femorale , replacement of artifi cial long-stem bipolar femoral head with bone cement.The clinical effect is excellent. 【Key words】 Stabilize Proximal Portion ; Intertrochanteric Fracture ;Aged Patients; Replacement of Artificial Long-Stem Bipolar Femoral Head |
