请问title page怎么写啊 已有2人参与

您好，我在提交论文修改稿时给我回复了如下内容 Please Upload a title page with the full names of authors, degrees, academic or professional affiliation, and complete address, phone number, and e-mail address of the corresponding author. Authorship statement must include as footnote in the title page that describes the contribution of each author (maximum one sentence per author). 请问这个title page是单独上传吗？ 格式和排版有啥要求吗， 一般哪些写在footnote ，哪些写在标题下面呢，真是搞不太懂，担心格式有问题又要重新搞。 然后就是Authorship statement 之前投稿也没要写这个，请问这个应该怎么写啊！ 很着急投出去，面临毕业早一天是一天，谢谢各位！ 期刊是 Elsevier 上的 computers & geosciences |
