Financial Relationships Disclosure Form 怎么写

昨天杂志给我回稿说是格式有点问题，让我改，可是第一次投稿，好多地方不太懂，还想请教一下大神。 There are details that require attention at this stage of the process; we apologize for the time it may take to attend to these: **The Title Page should also include the authors' degrees (e.g. DMD, MD, PhD). **JOMS requires that submissions include a completed AAOMS conflict of interest disclosure form for ALL AUTHORS listed on the title page before review can proceed. Authors must have a signed disclosure form included with each submission. *(Please have Dr. Su sign and complete their own disclosure form). 请问这个Financial Relationships Disclosure Form应该这么填写呢？电子签名是个什么东西···· 还有，这个1st 2nd前面需要打勾吗？我是一作，我的文章有1个通讯，还有个第二第三作者，没有并列的作者。 谢谢  68661273.jpg  56367258.jpg |
