Analysis of Common Problems in Pharmacy Intravenous Admixture Service of Certain Hospital

目的 分析在 静脉药物配制中心 过程中 常见问题 发生的 原因 以 保证 患者临床 用药安全、及时。方法 对我院 静脉药物配制中心 在2015年1月1日至2015年12月31日 期间发生的 差错 进行 统计分析，并提出 防范措施 有效减少 此类差错的 发生率。结果 及时的 差错分析 及 相应的 持续改进措施 显著降低了 静脉用药调配中心的 差错发生率。结论 有针对性地 采取防范措施 能够 有效地 降低静脉药物配制中心 差错发生率，可以 提高静脉药物配制中心的 工作质量和效率。
Abstract: Purpose: To analyze the cause for Common Problems in process of Pharmacy Intravenous Admixture Service and to guarantee a safe and timely clinical medication. Method: Perform statistical analysis for errors occurred in process of Pharmacy Intravenous Admixture Service from Jan 1st,2015 to Feb 31st, 2015, and propose prevention measures to reduce the incident rate of such errors. Results: A timely error analysis and corresponding continuous improvement measure can significantly reduce the incident rate of errors. Conclusion: Application of targeted prevention measures can effectively reduce the incident rate of errors in Pharmacy Intravenous Admixture Service and improve the quality and efficiency of work in Pharmacy Intravenous Admixture Service.
