血清肿瘤标志物在肺癌辅助诊断中的应用 目的 探讨5种血清肿瘤标志物在肺癌辅助诊断中的应用价值，并选择最理想的血清肿瘤标志物组合。 方法 应用酶联免疫吸附实验(EL1SA)检测170例肺癌患者、50例健康人和60例肺部良性疾病患者血清中神经元特异性烯醇化酶(NSE)、胃泌素释放肽前体(pro-GRP)、细胞角蛋白l9(CYFRA21—1)、p53抗体和癌胚抗原(CEA)的水平含量。 结果 肺癌患者的5种血清肿瘤标志物水平均明显高于健康人组和肺部良性疾病组，差异有统计学意义(P<0．01)。NSE、pro-GRP在小细胞肺癌中的水平明显高于其他类型的肺癌(P<0．01)，CYFRA21—1在鳞癌中的水平明显高于其他类型的肺癌(P<0．01)。p53抗体的特异性为100％，NSE、pro-GRP对小细胞肺癌检测的敏感性明显高于其他类型的肺癌(P<0．01)，CYFRA21—1对鳞癌检测的敏感性明显高于其他类型的肺癌(P<0．O1)。5种血清肿瘤标志物经组合后，敏感性明显高于任一单项肿瘤标志物(P<0．01)。 结论 5种血清肿瘤标志物对于肺癌的辅助诊断有一定的临床意义。NSE、pro-GRP二者可作为联合检测小细胞肺癌的首选标志物，CYFRA21—1、CEA和p53抗体三者可作为联合检测非小细胞肺癌的首选标志物。p53抗体对肺癌的辅助诊断有很高的特异性，CYFRA21—1对鳞癌的辅助诊断有一定的作用。 【主题词】肿瘤标志物；肺肿瘤／诊断；非小细胞肺癌／诊断 |

Application of tumor markers in serum in the assisted diagnosis of lung cancerPurposeExplore the application value of five kinds of serum tumor markers in assisted diagnosis of lung cancer , and select the best combination of serum tumor markers .Method we used the Enzyme-linked immunosorbent assay ( EL1SA ) to detecte the neuron-specific enolase (NSE) in serum , gastrin-releasing peptide precursor (pro - GRP) the cytokeratin l9 ( CYFRA21 -1 ) , p53 antibody and the level of content of carcinoembryonic antigen (CEA).All of above are from 170 cases of lung cancer patients and 50 healthy controls and 60 cases of patients with benign lung disease ResultThe level of five kinds of tumor markers in serum from lung cancer patients were significantly higher than the group of healthy individuals and benign lung disease group , the difference was statistically significant ( P <0.01 ) . The level of NSE and pro -GRP of in small cell of lung cancer was significantly higher than other types of lung cancer ( P <0.01 ) , the level of CYFRA21 -1 in squamous cancer was significantly higher than other types of lung cancer ( P <0.01 ) .The specificity of p53 antibody was 100 %, the sensitivity that using NSE, pro -GRP to detect small cell lung cancer was significantly higher than other types of lung cancer ( P <0.01 ) , the sensitivity that using CYFRA21 -1 to detect squamous carcinoma was significantly higher than other types of lung cancer ( P < 0.01 ) . After the combination of Five kinds of serum tumor markers , the sensitivity was significantly higher than any of the individual tumor markers ( P <0.01 ) . Conclusion Five kinds of serum tumor marker have a certain clinical significances for assisted diagnosis of lung cancer. NSE and pro -GRP can be used asr forof small cell lung cancer , CYFRA21 -1 , CEA and p53 antibody can be used as the preferred marker for joint detection of non-small cell lung cancer . p53 antibody has a high specificity for assisted diagnosis of lung cancer ,CYFRA21 - 1 has a certain effects on assisted diagnosis of squamous cancer 【Keywords 】 tumor markers ; lung neoplasms / diagnosis ; non - small cell lung cancer / diagnosis |
