I guess a lot of people have this problem, me myself included. Sometimes we do need to discipline ourselves a bit. Set a goal--very specific, like reading a paper, or a chapter of your coursebook and set a deadline--by which you should do nothing except working hard to achieve it.
on the other side, relax more when you are on holiday!

maybe you relax yourself for so long and cannot concentrate on your study immediately. Don't worry~ It's normal~
just one step at a time. spare some fixed time everyday for your work. don't do any other things until you finish them~

As a elder sister, I guess, maybe, although you have many plans for the coming semester, yet these things are not very urgent for you to carry out.
Clearly, I understood your feeling as I used to have it, , that, on one side you worry about your future tasks, and on the other side you have no mood to do anything.
It's normal for everyone, so, don't feel guilty. Sometimes, when I looked back my spent time, remember my suggestion, "Efficiency is the most important, rather than the amount of time, especially when the reading materials/books are necessary for your now-doing task.

corruption.   What about throwing your computer away,I think itis the most effective way to solve the problem.
Take it easy,it doesnot matter,everyone has such problem,.You may try try draw up a work schedule,and control the time for idle affairs.
