Usually the DS-2019 is a form that I fill out here and submit to our International Student & Scholar Services who will process it and send you a formal statement that you can use for your Visa.  I will need the following information

1. Country of Citizenship

2. Country of Permanent Residency

3. Previous time in J Status (Dates if you had a previous J status and what institution)

4. Are you planning on bring any dependents
If so I will need they names, relationship, date of birth, city and country of birth, country of citizenship and country of permanent residence.


Also your certified proof of funding needs to show that you are able to fund yourself in the amount of $1,550.00 per month.
